Underhood Label Instructions

UNDERHOOD LABEL INSTRUCTIONS:

1. Raise the hood and properly support it with the hood prop rod.







2. Locate the open flat area where the label will be placed.

3. Clean the surface with isopropyl alcohol or glass cleaner.




4. Lift the bottom flap* on the "Authorized Modifications Label" and write the following information:

- CHANGE AUTHORITY: 035

- DEALER CODE: Your Dealer Code

- REPAIR DATE: The date of the repair

NOTE: *This is a custom, two layer laminated construction label. The top layer is partially laminated to the bottom layer. The flap (bottom portion of the top layer) is lifted to expose the information fields mentioned above.

5. After writing the information from step 4, remove the backing from the bottom flap to expose the clear adhesive. Next press the bottom flap down to laminate it to the front of the label where you just entered the information.

6. Next remove the backing from the label, and press the entire label on the engine compartment wall in the position shown in step 2.

CUSTOMER INFORMATION TAG

Hang the Customer Information Tag for Campaign 35 on the rear view mirror after completing the reprogramming and installing the Underhood Label. (See illustration and photo below)





VEHICLE EMISSION RECALL - PROOF OF CORRECTION FORM

A Vehicle Emission Recall - Proof of Correction form must be filled out for all cars registered in California and Massachusetts.

California and Massachusetts residents who own an affected vehicle will be required to possess a Vehicle Emission Recall - Proof of Correction form as evidence of recall completion when they apply for vehicle registration renewal. Each customer whose vehicle is registered in California or Massachusetts must be given a completed Vehicle Emission Recall - Proof of Correction form after the recall has been completed.

NOTE: This form MUST be completed by the Dealership's Service Manager using a permanent ink pen. A fine point pen is recommended. When filling in the information, copy the information directly from the vehicle, NOT the repair order. ALL INFORMATION MUST BE PRINTED AND NEAT.

DO NOT COMPLETE THE FORM UNLESS THE RECALL WORK IS COMPLETED!

Instructions:

1. Enter the vehicle's license number

2. Enter "Hyundai".

3. Enter the vehicle's Model year "1999".

4. Enter "Sonata".

5. Enter the Vehicle Identification Number (VIN)

6. Enter "035".

7. Enter your dealership name.

8. Enter your dealership's physical address. (Not the mailing address.)

9. Enter date of repair.

10. Sign the form.

The copy of the form shown below indicates the areas to be filled in. Each area must be filled in.
